Guidance Appointment Request >	This form should be used to request an appointment.  Sometimes you can email your counselor if you have questions too.
Course Add/Drop >	Please see your Guidance Counselor to Drop, Add, or change levels in a class.  **See Add/Drop Policies**
Release of Records >	This form should be used by all former students or by current students who are withdrawing or transferring to another high school.  (new form coming)
College Visitation Day Pass >	Use this when colleges are coming to visit Pinkerton.  Your teacher must approve your visit and sign your pass.
**Mandatory College Application Forms**
Transcript Release Form >	Used to send transcripts to colleges, should be given to your counselor with each school you are applying to at least 30 days before college application deadline.  ONLY colleges you have or are going to apply to.  It will allow your counselor to send:  Transcripts, Letters of Recommendation, School Profile
Teacher Information        & Recommendation Forms >	Most colleges require recommendations from teachers.  Students should fill out these 2 forms and ask their teacher to write them a recommendation.  Both forms must be complete for a teacher to write a recommendation.  Teachers will send their completed recommendation to guidance.  It is the students responsibility to make sure guidance has received their recommendations, we do not know who they are asking to write a recommendation for them.
Counselor Information Form >	This form in necessary for your counselor to write a recommendation for you.  It helps your guidance counselor learn more about your personality, achievements, and college interests.
Scholarship Release Form >	This form gives Pinkerton permission to send your transcripts to scholarship organizations.  Be sure to attach any needed documentation to this form that Guidance would not provide such as college acceptance letter, essay, outside recommendations, etc.  Also, indicate information that Guidance must include and if specific information on the scholarship needs to be filled out by your counselor.
